Create Hamilton Questionnaire

This questionnaire has a form and it is necessary to store its answers and as well compute the final score of the questionnaire. There is the possibility to compute the score on AWS before storing the answers on the database.

This questionnaire has a period of 3 months.

The doctors have the possibility to visualize the previous questionnaires.
