The falls diary should question if the patient has fallen and if had injuries (check the table in the link, the questionnaire does not need to be in the table format, it is just for guidance).

for now, let's consider this can be filled monthly, so it should allow for feedback for the previous month.

Additional questions should be answered for the socioeconomic analysis (if there are falls):
3.1. Did you need to schedule a visit with your specialist as a result of the fall? If yes, could you please specify the total number of visits made?
3.2. Did the fall necessitate hospitalisation? If yes, could you provide the duration of the hospital stay in days?
